I tried using two real Wiimotes in Mario Kart Wii in the latest revision (r6107 fixed the bug with the uninitialized "index" var), with both Wiimote slots set to "Real Wiimote". I had to go to Tools->Connect Wiimote 2 (ALT+F6) before the game (and the menu that appears upon pressing the Home button) actually recognizes the second Wiimote as connected.

Also check your wiimote settings so that you are using 2 different udp port numbers.
UDP settings are not important if you are using multiple real WiiMotes directly connected. Just make sure everything is setup:

- connect all WiiMotes in your BlueTooth stack (Win7 native works OK)
- set all WiiMotes to 'Real WiiMote' in Dolphin
- start the game with the first WiiMote
- once game is running with one WiiMote connect all other WiiMotes using menu or ALT+F6 (2nd), ALT+F7 (3rd) or ALT-F8 (4th)
- Dolphin should give feedback about connection in yellow text in the upper left
- WiiMotes should indicate the correct number with the blue LEDs

It is working here with 2 WiiMotes. Sometimes I have problems activating all WiiMotes, then just stop Dolphin, check WiiMote connection in you BlueTooth stack (maybe remove WiiMotest there and reconnect) and start over.
