yeah, if youre using clone CD images you have to select "run cd" meaning the virtual drive that windows thinks is a CDrom drive. automatic configuration of the cd plugin worked perfect for me. If it works with the cd, as you said, it will very probably work with the "virtual clonedrive" of clone CD.

as far as i know its imposssible to speed up the iso loading process that epsxe does. however by having a saved state right after the in-game loading, and intro crap etc. you can save yourself some time especially on games like samurai showdown which has a bunch of "loading" screens. just save after the intro once, and then you can reload that state every time you want to play afterwards.

It depends, Cd-rw are often slower. Mine for example is 8x4x32x (32 is the speed at reading cds) and my dvd is 16x (and reads cds at 48x). Just consult your manuals.
 
If you're using ISOs, then the problem is in how fast your graphic card + processor + memory can render the game [mention here using ePSXe].

Make sure you've activated DMA for your HDD.
 
The read numbers concerning media drives can be confusing and deceiving as well.

For example, my 8x DVD-ROM can only extract digital audio at 4x, where my 24/10/32x CD-RW can extract it at 8x. Now, I KNOW that's important when copying CDs and such, but it may factor into the overall loading/reading time with PSX CDs as well.

In general though, use an ISO if you can make a playable one and have enough space.
