oh, there's a lot more Sega than what I've seen listed so far ;)
I'll try to list what hasn't been listed so far, but apologies on double entries...
also, I'll try to post mostly stuff from the "NGEmu" era and beyond (e.g. Saturn, PS and what came after that), but also a few 16-bit titles that arrived very late on the PC. I'm also not listing DOS games. ;)
And this list isn't complete either, just what I remembered and what I have =P
about the origins: this means where they were ported from, not where the original game appeared on first. For example, most Saturn games are in turn "ports" (or reprogrammed) from Arcade Hardware, but the PC versions were all straight Saturn ports.

Sega

Daytona USA (origin: Saturn)
Daytona USA Deluxe (origin: Saturn)
Sega Touring Car Championship (origin: Saturn)
Sega Rally 2 (origin: probably Dreamcast)
Sega GT (origin: Dreamcast)
Sky Target (origin: Saturn)
Panzer Dragoon (origin: Saturn)
Sonic CD (origin: Sega CD)
Sonic & Knuckles (origin: Megadrive)
Sonic 3D Blast (origin: Saturn)
Sonic R (origin: Saturn)
Sonic Adventure DX (origin: GameCube)
Sonic Heroes (origin: it's renderware =P so maybe technically not a port?)
Sonic Riders (origin: unknown, prolly same as Sonic Heroes)
Crazy Taxi (port from the ps2 port of the dreamcast version =P )
Crazy Taxi 3 (origin: Xbox)
Virtua Fighter 1,2 (origin: Saturn)
Virtua Cop 1,2 (origin: Saturn)
Last Bronx (origin: Saturn)
Formula Karts (origin: Saturn)
House of the Dead 1 and 2 (origin: Saturn and Dreamcast)
Outrun 2006 (origin: dunno, but I think (hope!) Xbox)

Capcom

Megaman X series (most of them, but there might be missing a few, origin: PS1, PS2)
Street Fighter Alpha 1 and 2 (origin: unsure, could be PS, or maybe even CPS-2)
Super Puzzle Fighter 2 Turbo (origin: unsure, could be PS, or maybe even CPS-2)

Konami

ISS 3 (origin: PS2, I think)
Dance Dance Revolution (sort of a remake, technically not a port?)
PES (forgot which ones) (origin: PS2)
Gradius Deluxe Pack (Japan only) (origin: probably PS1)
Turtles (the new games, dunno if all of them were ported) (origin: Unknown (Cube???))

SNK

Fatal Fury 3 (origin: probably NeoGeo CD)
Samurai Spirits (origin: probably NeoGeo CD)

Taito

Layer Section (RayForce/GunLock) (origin: probably Taito-F3)
Ray Storm (Layer Section 2) (origin: PlayStation)
Darius Gaiden (origin: probably Taito-F3)
Puzzle Bobble/Bust a Move series (several, published by different companies, ported from different versions)

Other

Raiden 2 (origin: probably based of the arcade version)
Raiden 3 (Japan only, so far, hopefully a Taito Type-X port and not a PS2 one)
Le Mans 24 (port from the ps2 port of the dreamcast version =P )
Midnight Club 2 (origin: PS2)

they made a pc version of VIII? i knew about VII's pc port but i didnt know they did one for VIII too. why did they stop at VIII then and not do IX as well?

guess thats probably why i never heard of it.

Hmm, I remember more....

Virtual On (I loved this game, how could I forget it? Sadly, it doesn't work on modern processors and Windows as well.... :( )
Tomb Raider series
Enemy Zero (PLEASE! SOMEONE HELP ME FIND THIS THING! I'm quite sure this game exists in the PC! :( )
Twisted Metal 2
7 Sins
Prince of Persia series (The Sand saga games specifically.)
Alone in the Dark : The New Nightmare
Beyond Good and Evil
Breath of Fire 4
Megaman X3, X4, X5, X8
Destruction Derby
Scarface : Own the World
Lego Star Wars
Mafia
Onimusha 3

can some of those games still be considered ports though? For example, I didn't mention the NFS series since in that case it's more multi-platform than that they're ports (allthough the very first one can probably be classified as 3DO port). It's kinda hard to exactly define what a port is, though... uhm, is there a defenition for it?

Oh, I remembered another:

Hydro Thunder (and I don't know which version it was ported from)

well NFS wasnt ported to PC, that was made for each platform, the only difference is it seems next gen consoles (like the Ps3) are getting a port from the PC from what i can tell ;p

in my mind

Port = Game originated from another platform which has been recompiled to run on different hardware. DMC3, GTA3 and RE4 being good examples. Most of the time its not a direct port but features are added/taken away to the developers disgression, like GTA3 had mp3 playing added and improved graphics over the PS2 version (WTG Rockstar!) then you get *****es like Crapcom who take a perfectly good game and butcher it cos they cant be bothered in doing a proper job.

thats my definition ;p

Mafia was a PC game ported to consoles.

Anyway, from my own DC collection:

Omikron: The nomad soul
Re-Volt
Fur Fighters
Evil Twin (might as well be a pc -> DC port)

Well, that seems about how I think of it. I guess you're right on NFS, I was just wondering if the first was reprogrammed as a PC game or a 3DO port.... it was probably reprogrammed.
By the way, there also exists a version of Super Street Fighter 2 Turbo on PC of which it could also be doubted if it can be classified as a port, as an incredible amount of the game was redone (by Gametek). It ran in 320x240 (or 320x200, not sure) but retained the original arcade sprite sizes. Music was redone in Midi, but the CD-Rom version had CD Audio music.
