Check everything except the realtime config window unless you'd like to have that as well, and I recommend a buffer length of 6-8 and an xa buffer length of 48, but you'll have to experiment a bit to find out what works best for you, also make sure you don't have the spu irq hack enabled inside of epsxe itself. Read the translated documentation.

Should the other 4 boxes be checked in the sound menu?

I'm trying to play both FF4 and FF5 but the sound is just to bad.

Yes the other 4 should be checked. Can't help you with ff4 & ff5 other than suggesting that you run them under a snes emulator, and just download the fmvs to watch.
