pcsx2 crash

hey everyone, having a hard time getting pcsx2 to run,weird thing is I was fine till I had to reformat I had pcsx2 running golden and now it just keeps crashing even though I set it up exactly how I had it running before heres a ss of the crash report and my gpu plugins if I need anymore to help determine what might be wrong please let me know.Any help would be appreciated thanks.
