Video, though most likely it's an issue with your current settings. Sound plugin likely could use some tweaking too since the default plugin choice and the default settings are less than optimal.

When I use Pete's D3D Driver 1.74 the game runs fine but the swirl randomly works.
As for the sound, ePSXe SPU core 1.7.0 works perfectly.
